- Chair letter explaining the basis for continuation of the faculty member's contract and a description of departmental faculty committee deliberations
- Faculty member's annual review** and academic plans and measurable outcomes both must be signed by the department chair and the faculty member

- Service Request (if past payroll deadline)
- Electronic Employment Eligibility Verification Form (I-9) Instruction(if applicable)
THE COMPLETED EMPLOYEE RECORD (ER) MUST BE SUBMITTED WITH ORIGINAL AND ONE COPY OF ALL SUPPORTING DOCUMENTATION **If the faculty member's prior contract was for 2 or 3 years, the annual review for those years must accompany the current annual review.
