Administrative Checklists: Faculty Leave For Professional Enhancement/Unpaid Leave

  1. Completed Employee Record (ER)
  2. Faculty member's written request, stating purpose of leave and requested time period
  3. Letter of approval from departmental chairman to contain:
    • Endorsement
    • Assurance of coverage for duties and responsibilities of the faculty member
  4. Copies of up-to-date curriculum vitae and bibliography in proper format
  5. Service Request (if past payroll deadline)

THE COMPLETED EMPLOYEE RECORD (ER) MUST BE SUBMITTED WITH ORIGINAL AND ONE COPY OF ALL SUPPORTING DOCUMENTATION
