  5. Certificate of English Language Fluency for Teaching
  6. Curriculum Vitae
  7. Faculty Email Form
  8. Copy of Offer Letter (Faculty paid by UPP Only)
  9. MD License (Faculty paid by UPP Only)
  10. Indicate salary in "Remarks Section" (Faculty paid by UPP Only)

NOTE: Appointments at the associate and professor levels require review by the appropriate School of Medicine appointments and promotions committee.

THE COMPLETED EMPLOYEE RECORD (ER) MUST BE SUBMITTED WITH ORIGINAL AND ONE COPY OF ALL SUPPORTING DOCUMENTATION
